We welcome new referrals for adult and senior patients!  
Medical/ Professional referrals: 

Please fax a completed copy of our referral form with a progress note, neuroimaging reports, laboratory results, and copy of the patient’s insurance card to  949-862-8661.

What Should I expect?

Session & Report Timelines

Patients are seen in-person for a 4-hour combined interview & testing session. A 30-60 minute telehealth feedback session is held within 2 weeks to review results, diagnostic impressions, and recommendations.

Patients and referring clinicians receive a written report approximately 1 week after your feedback session.

What Should I expect?

Get Ready for Your Appointment

Before Your Appointment - Information:

Please complete the above Intake Paperwork forms prior to the date of your visit and submit them via this email or your patient portal (if applicable).

On the Release of Information forms, please include relevant phone and fax numbers for your referring medical provider. If you have had MRI or CT scans completed, a release should be completed to obtain those records, as well.

Our billing specialist will complete a verification of your insurance benefits.

Before Your Appointment - Medication

If you are currently taking medication for Attention Deficit Hyperactivity Disorder (ADHD), please do not take your medication within 24 hours of testing.

If you are currently taking medications that cause drowsiness, please do not take your medication within 4 hours of testing.

Otherwise, please take your prescribed medications. We suggest that you have a good night’s sleep and a good meal before your office visit. In addition, please avoid excess caffeine and sugar consumption.

On the Appointment Date, you will need to bring:
  • Your insurance card
  • Photo ID
  • If available, copies of the report from any prior neuropsychological evaluation and any medical documents related to the difficulties you are experiencing.
Please note that you cannot be seen if a copy of the front and back of your insurance card is unavailable.
To Reschedule:

If you are unable to keep this appointment, please call us at least 24 hours prior to the appointment and we will reschedule a more convenient time for you. Late cancellations (less than 24 hours notice) may result a $250 charge that is not covered by your insurance company.
